It looks like there are two different versions of this song. Parts in green appear in all versions. The blue section appears in the piano version (and presumably the original version, too), whereas the red sections appear in the ‘FINAL’ version of this song.

散りゆく (chiri yuku) is quite a poetic term, commonly used in the context of blossoms ‘falling.’ Here, I think it’s being used in a metaphorical way to talk about the speaker’s death.
